Through our Pathways program we guide individuals, family members, caregivers and partners on what resources and funding are available in our community, and at our future campus. In simple terms, think of us as a central point through which to gain access to the necessary support and care. 

Whether you are looking for a support group or workshop through we you can join with experts and peers in addressing the day to day challenges of living with a disability, OR you need access to therapy and case management services, we want you to think of Collaboration Center Foundation as your one and only destination.
