Our Care Transition Team will talk to you prior to discharge to explain our services and to answer any questions you may have.
We are available to you 24 hours a day, 7 days a week. A nurse is only a phone call away.
When Medicare is your insurance, services are provided free (no deductibles or co-pays).
A first visit by your Case Manager- a Registered Nurse or a Physical Therapist.
Your Case Manager will work with you, your Family and your Healthcare Provider to develop a plan for your care.
Your care plan will be explained to you and carried out by your healthcare team, which may include: highly-trained and compassionate nurses, physical, occupational, speech therapists, social worker and caring home health aides.
